If you are interested in becoming a OneCare Collaborative physician partner, please direct your request to OneCare Collaborative Administration at 812-426-9402. Our Credentialing Committee reviews all provider requests for participation in our ACOs.

IMPORTANT DETAILS
There are a few important details to remember about the physician’s agreement with the OneCare Collaborative:
- You can terminate your agreement with us at any point by providing a 90-day termination notice.
- Once you are contracted with us, the agreement supersedes any existing risk-sharing or gain-sharing agreement with our participating payers.
About OneCare Collaborative
The goal is to support physicians as they care for patients by making sure they have the most up-to-date information about patient health history and activity. For patients, this means doctors communicate better with each other, can avoid ordering duplicate tests, and can connect patients with a dedicated nurse Care Advisor who can provide additional support and education for the patient in-between doctor appointments.
Physicians participate in the OneCare Collaborative because they are committed to providing patients with a better care experience. They may also be rewarded for offering highly coordinated care with the help of OneCare Collaborative care team resources, including:
